Need Help Accessing Your Immunization Record?
If this is your first time using the portal or you're unsure whether your information is up to date or correct, we can help. Start by following the steps below.
To Access Your Record:
- Select who you're requesting for (yourself or a dependent).
- Enter the patient’s:
- Full name
- Date of birth
- Gender
- Email address
All details must exactly match what we have on file.
- If a matching record is found, the system will email you an access code.

Need to Update or Verify Your Information?
If the portal says no record was found, or you're not sure what information is on file, please contact us.
Email iis@chcc.health and include:
- Whether you're requesting for yourself or a dependent
- A short note describing the issue (e.g., “no record found,” “need to update info,” or “locked out”)
We’ll follow up with the next steps and let you know what information or documents are needed based on your request. Depending on the situation, we may ask for additional documentation to verify identity or your relationship to a dependent.
Locked Out or Received a “Search Limit Exceeded” Message?
This usually means your searches didn’t match what’s on file and access has been temporarily restricted as a security precaution.
